Forgotten your PhilHealth PIN, MDR & Validity? No need to fall in line, get it online!

The Philippine Health Insurance or ‘PhilHealth’, as what it is known to many Filipinos is a national health insurance program of the Philippine government.

Once registered as a member, you’ll be assigned a Personal Identification Number (PIN).

You then use this PIN to pay the required membership contributions. Obviously, if you used an incorrect PIN when paying your contribution, another PhilHealth member may benefit from your hard-earned money.

As what is posted on its website, the agency reminds us,

“Siguraduhin tama ang iyong PIN upang hindi maitala sa ibang miyembro ang iyong hulog.”

Moving on, you can check your latest date of payment, and the validity of your health insurance through the Member Data Record (MDR).

Take note that in times of confinement, some hospitals may still require you to provide a hard copy of  MDR for your expenses to be covered. These hospitals most likely don’t have access to PhilHealth’s ‘My PhilHealth Portal’ facility.

Nevertheless, you can always secure a personal copy of your MDR. Read on, we will teach you how.

How to get MDR

There are two ways to get an updated copy of MDR.

1.) Get a copy from any PhilHealth office near you

2.) You may request a copy to be sent straight to your email.

Here’s how to request for an electronic copy.

a. Open your email and write a message to ofp@philhealth.gov.ph

b. In your message, write your correct and complete:

• LAST NAME
• FIRST NAME
• MIDDLE NAME
• BIRTHDAY

c. Send your message and wait for PhilHealth’s reply!

d. Once an electronic copy of your MDR was emailed, print and keep a copy of it for future use.
